After receiving assessment feedback, update the plan of care as needed. 2. Meet with older adult and provide teaching and implementation of the amended plan of care. 3. After two weeks, meet with the older adult and evaluate any change, effect or feedback regarding the care interventions/teaching. Consider nursing process evaluation in this step. 4. Write a paper demonstrating the plan of care updates, discussion to the implementation process include any teaching materials you used and the outcome of the implementation. Consider the effect of any changes the older adult experienced or not. Also consider the older adult’s feedback or comments positive or negative. Address any changes to the plan of care you feel should be improved this older adult. What would you do differently and provide rationale/support for change?
