Vulnerable Population: Minority women ages (19-35?) without health insurance
This is the vulnerable population we picked but if nothing found age can be changed.
Will need in text citations for each questions same source can we used across questions reputable
websites can be used as well and at least 3 peer reviewed articles.
1) Population overview: Demographics (age/gender/ethnicity/income/education levels, etc.)
2) Health indicators, including data and statistics identified through your research. What are the social
determinants of health for your chosen population?
3) What are the health issues of the vulnerable population
4) Pick at least one health issue that the vulnerable population faces and focus on addressing this health
issue.
Use the Healthy People 2020 website to help you narrow down what the group is going to address.

5) Write a SMART goal of what the group wishes to accomplish. Use attached smart goal worksheet and
definition as reference.
SMART goals are:
• Specific: Well defined, clear, and unambiguous
• Measurable: With specific criteria that measure your progress towards the accomplishment of the goal
• Achievable: Attainable and not impossible to achieve
• Realistic: Within reach, realistic, and relevant to your life purpose
• Timely: With a clearly defined timeline, including a starting date and a target date. The purpose is to
