Introduction – Identify and briefly describe a quality improvement project or initiative at a clinical site or possibly where you work.
Background – Describe the reasons or need for the project and provide background information on the inception. Identify stakeholders in the initiative and the projected outcome related to patient safety, patient outcomes, quality of care, and/or nursing practice. Include target or benchmarks and where the benchmarks were derived from.
Research and Evidence – Support the project or initiative with a minimum of two scholarly resources showing evidence to take the proposed action.
Methods, Resources, and Performance Targets – Describe how the project was put into action, responsibilities of team members including nursing, resources utilized, discuss strengths, weaknesses, opportunities, and threats (SWOT).
Results – Discuss the results of the initiative including impact on quality of care. What are the plans for this project or initiative? Will the work continue or be modified?
