In this paper, you will focus on ONE of the “health needs” you identified in Part I.  (NOTE: You will also design a brochure to address this need in Part III). • A minimum of 3 three scholarly sources written within the last 5 (five) years is required.

Identify a “Health Need” which relates to your community from Part I of your Community Assessment. How does this “health need” align with the Healthy People 2020 objectives (www.healthypeople.gov)? Complete a literature review that relates to the “health need” you identified.  Discuss the significance of the problem and relationship to the overall well-being of your community and support this problem with data collected from your community. Answer questions such as: Which problems are important to the community? Which segments of the population are most affected? What are the benefits to the community? What happens to the community or the population if the problem is or not resolved? How do politics, community values, and community priorities affect efforts to solve the problem? What does the community expect to happen? Once the high priority problem has been identified, relevant goals and objectives are developed.

PART THREE: COMMUNITY HEALTH EDUCATION BROCHURE: (due by Week 6 / Presentation to Community due by Week 8)

  • You will select an agency in the community for implementation of the community health education brochure. Contact the community agency to identify a person/s who you can develop the brochure for. The health topic selected for the brochure must be a health need for the community. • The Community Health Education Brochure will be developed and submitted to the instructor – due week 6 (See Grading Rubric for Brochure). • Once the Community Health Education Brochure is developed and approved by the instructor and the contact person at the community agency, you will present it to a community group – due week 8. The practicum verification form, included in this syllabus, must be submitted to complete this assignment.

 

Examples of community recipients may include: health fairs/screening programs; senior wellness centers; churches; schools; or other voluntary community agencies. The community agency must be approved by the instructor, and contact information for the agency representative must be provided by week three.

Guidelines:

  1. Determine a teaching concept in which a brochure could be used to reinforce the understanding of the chosen concept.
  2. Use any software available to develop a 3-panel teaching brochure using Patient Education Guidelines for Education Materials
  3. Readability level must be at the “6th grade level” or lower.
  4. Include graphics and color to enhance an idea or concept. Be sure graphics are licensed for reuse and sharing.
  5. Write information in a font size of 16 or larger.
  6. Proofread brochure for typographical and grammatical errors.
  7. Ensure information is accurate and up-to-date.
  8. Organize content in a logical, step-by-step fashion and present information concisely.
  9. Avoid medical jargon. Define medical terms as necessary.
  10. Write in conversation style. Keep words and sentences short.
  11. Use second person (you) and write in the active voice.
  12. Build in reviews at the end of the brochure.
  13. Use plenty of white space.
  14. State ideas/suggestions/directions/instruction in a positive manner.
  15. Present powerful information tactfully.
