Create a logic model outlining your program, specific goals and objectives, and specific program inputs (i.e. who, what, where, when, why). Please get as specific as possible with details of your plan, including partners/stakeholders, resources needed, specific program activities, etc., and design your logic model in a way that could be easily translated to a potential partner. Your logic model should be a visual representation of what you plan to do and should link program goals related to your health issue to specific program activities and desired outcomes.
Write a reflection explaining your model step by step and discussing/justifying the theoretical assumptions that underlie your program. What do you hope to achieve and what are some basic ways to measure your achievements? What theories are most helpful for you in designing your program (and why)? What are some limitations or criticisms? How might you adapt or modify existing approaches for your specific target population? Who will be involved in this work and in what capacities? What resources are needed (try to be more specific than “money” or “funding”)? What are some ways you might go about tracking program success?
