    Any handout or presentation used for the implementation (e.g., information sheet, rubric, etc.)
    Site approval letter (one or two short paragraphs giving permission from principal, director, etc.)
    Internal Review form only if you are conducting research as a Keiser employee at a Keiser campus
    Note: if you have trouble submitting these two separate documents in the assignment area for week 6, submit the IRB App as the assignment and email the
    Add'l Docs document to your professor. You will then continue to work with your professor until the application is in its final form and approved by your
    professor to be submitted to the IRB Committee (DO NOT SUBMIT DIRECTLY TO THE IRB AS YOUR PROFESSOR WILL SUBMIT ALL OF YOUR DOCUMENTS
    WHEN READY FOR REVIEW).
    Also note that the single Word doc is for purposes of working with your professor to get all items in final form and to eliminate the submission to your
    professor of multiple documents. Prior to submission to the IRB next week, you will create a pdf document for the IRB app and add'l docs so everything is on
    one pdf document (instructions are in week 7).

    IRB Application (55 points):
    / 5: Principal Investigator/Title of Study/Degree Program and Proposed Dates are properly completed. /40: Reflective and critical thinking (i.e., application, synthesis, analysis, and/or evaluation) are coherently and consistently used to describe the overview
    of the study, research questions, methodology, purpose, requirements, data courses, access, and subject specific jargon.
    /5: The Study Population and Procedures are clearly identified and easily replicable. /5: Methods for protecting confidentiality and document storage are clearly explained.
    Single-Word Doc of Add'l Docs for IRB (35 points):
    /5 Your CITI Certificate of Completion (Completion Report in pdf format) /5 Informed Consent (filled out as it will be received by the participant to sign – see examples)
    /5 Data Collection Tool -- Survey Questionnaire or focus group questions that will be used pre and post exactly as they will appear as well as /5 Copy of email or scriipt for requesting participation
    /5 Any handout or presentation used for the implementation (e.g., information sheet, rubric, etc.) /5 Checklist (required by all) students and Site approval letter (one or two short paragraphs giving permission from principal, director, etc.; only for those
    who require site approval letter from the appropriate administrative authority on letterhead).
    /5 Bio & Role form (required by all students) and Internal Review form (only if you are conducting research as a Keiser employee at a Keiser campus). Quality of Writing (10 Points) / 5: Precision – Student uses terminology and writes clearly and concisely.
