3. Please read the article on Institutionalizing Quality Assurance located in this week’s assignment folder; It is six pages in length. This is an older article (2002) but the framework is the one used by many organizations to institutionalize quality assurance efforts.

a. In the article, Across the Chasm: Six Aims for Changing the Health Care System, the author points to six aims for improvement. Choose one of these aims and discuss the following: 1. Define the Aim 2. Describe how this Aim applies to health care quality 3. Is health care quality improving — yes or no? Support your answer with facts. 4. Provide at least two quotes from this module’s articles to support your position
b. In Chapter 12, discuss one of the scenarios in detail, and describe the following 1. What is the issue? 2. Discuss the Systems Factors that negatively affect the scenario. 3. If you were designing a QI Project to address the issue, what would be your Indicator(s)? 4. What is/are the Root Cause(s) of the scenario? 5. c. In Chapter 13, discuss the Elephant of Error by Analyzing an Adverse Event. 1. Describe one adverse event from your experience in relation to the discussion from the text. 2. What could have prevented the adverse event? 3. What actions could prevent recurrence of a similar event? 4. Properly reference and cite from the text and any other sources you use.
