Case Scenario: John is a 60-year-old male who has been diagnosed with Post Traumatic Stress Disorder
(PTSD), Bipolar 1and Substance Abuse Disorder. His mother was also diagnosed Bipolar when she was 70
years old. John was raised in a home environment where physical violence occurred frequently both within the
home and outside of it. John graduated high school at the age of 15. When he was 18, he joined the United
States Army and after basic training, was shipped off to Vietnam where he served for two years. While in
Vietnam his best friend in the service was killed in front of him. Upon release from the service, he received no
mental health counseling and turned to drugs and alcohol. John has spent a number of years in prison for petty
crimes. He has been suicidal on a number of occasions and has spent time in both dual-diagnosis inpatient
treatment hospitals and centers. When taking his prescribed medications, he is calm and leveled out. However,
he will not take them as he does not like the side effects of them. To date he has not given up his drugs and
alcohol.
